It’s Christmas season in New York City, and Otto (an attractive young writer) is out to lunch with Naomi, his badass, tattoo-artist girlfriend. He gives her an early Christmas gift…an adult “toy.” It’s a small piece that she can wear between her legs that vibrates. Otto can control the vibrations from an app on his phone. Naomi, who has been itching for more sexual adventure, is thrilled with her present. The two plan to attend a concert later that night, and Otto wants her to wear her new item to the concert.

Meanwhile: in Long Island, Diego (Otto’s best friend) is working a hospital shift. He’s a resident, and he’s going through Otto’s files. Otto is a frequent flyer of the hospital for two reasons: 1) Otto has a chronic kidney condition and has already had one transplant, and 2) both of Otto’s dads work at the hospital. Diego works with Otto’s dad, Donovan, who is the disgruntled CEO. During his shift, Diego finds that Otto’s blood test has come back abnormal. He alerts Otto, and Otto pleads with him to keep the results to himself. Reluctantly, Diego agrees not to tell Otto’s father. Otto invites Diego out to the concert tonight, and Diego accepts.

The three of them attend the concert. During the show, Otto plays with the toy, bringing Naomi to the edge of orgasm over and over again. When Diego realizes something is going on, Otto tells him about their “game.” Then Otto hands his phone over to Diego and invites the other man to “join the fun.”

Naomi and Otto take the dance floor. Meanwhile, Diego uses Otto’s phone to control Naomi’s toy. Playing with both of them drives Naomi wild and she has an orgasm on the dance floor.

Diego and Naomi rush Otto to Hannsett Island Hospital, where Otto’s fathers work. They discover that Otto has been hiding his condition for a long time and his transplanted kidney is now completely failing. He’ll need a new kidney — and fast.

However, Otto refuses a new kidney. He’s stubborn and convinced that, since he already had a transplant, he shouldn’t be forced to get another.

His refusal puts a strain on the family. Otto’s parents arrive at the hospital: Jason, Donovan, and Kenzi. It’s revealed that the three parents have been in a crisis. Diego’s mother, Maria, passed away recently due to cancer. The two families were incredibly close and the grieving process put an intense strain on their relationship — particularly, it put a wedge between Jason and Donovan. Donovan has been sleeping at the hospital so to avoid confrontation. The news of Otto’s sickness brings them together for the moment, though there is clearly tension there.

Jason and Donovan get in a heated argument over Otto’s treatment plan. Arguing turns into lust and the two ravage each other in Donovan’s office.

Meanwhile, Otto and Naomi argue in his hospital room. Naomi is angry that he won’t accept the treatment since she planned a future with him. Otto remains stubborn and Naomi leaves, hurt and mad.

Naomi heads to the ferry to leave Hannsett Island. She runs into Jason, Otto’s dad, who sits with her to wait. The two have a touching conversation. Naomi looks at him and recognizes Otto’s features in his face. Heartbroken, she leans forward and kisses Jason. Jason pulls away; an awkward moment between them.

Eventually, Naomi decides to stay on the island, and goes to Diego’s place. Jason returns to the hospital and confesses the kiss to Kenzi, who finds the whole thing amusing. Kenzi, Jason, and Donovan share a moment and, for a second, there seems to be a chance at reconciliation.

Otto falls asleep and has a dream about his family. He realizes that, as much as it hurts him to give up his kidney, he has to do it in order to stay alive. He loves Diego and Naomi too much to leave them.

When he wakes up, his sister, Joan, has arrived. She’s a kidney match and the two go into surgery so she can give him the life-saving kidney.

Later, it’s New Years Day. Donovan, Jason, and Kenzi come together again. Donovan apologizes for being so distant and expresses gratitude to his partners for giving him space. The three of them share a steamy, intimate moment in the lighthouse.

Otto, Diego, and Naomi are celebrating a belated Christmas together. Otto is still recovering, but the three of them find creative ways to be intimate with one another.

The story ends with both trios enjoying a spicy, happy ever after. The door is still open, however, for Joan, who will find a trio of her own in the final book in the series, The Pink Lantern
